Lost my physical health insurance card – how to check if it's still active?

Hey everyone! I have a bit of an awkward situation. I’m scheduled for a dental appointment tomorrow morning, but I realized that I’ve completely misplaced my physical health insurance card during my recent apartment move. My company changed our insurance provider a couple of months ago, so I don't even remember the exact name of the new network or my policy number.
